At Physical Evidence (Chiropractor in Boca Raton), we understand that there are many factors and layers to the cause of musculoskeletal pain including back, neck, jaw (TMJ), shoulders, elbow, wrists, hands, hips, knees, ankles and feet. Unlike most practices, we address all spinal and extremity related conditions as we realized long ago that the body is one completely interactive mechanical system and must be viewed globally in terms of correcting all of the dysfunction which causes pain.

Our treatments all include a combination of therapies, both manual and technological, that address the biochemical (inflammation), biomechanical (muscles spasm & joint dysfunction), and functional (muscles weakness and imbalance) aspects of the dysfunction associated with pain.

Biochemical

Inflammation is the main component of pain as it stimulates specialized nerves that provide the brain with the biochemical stimulus which the brain interprets as pain. In the body that usually takes the form of tendonitis, bursitis, myositis, and joint swelling. Our primary technology which reduces inflammation rapidly which we administer is our Aspen Apex Class IV Deep Tissue Laser. The Apex sends 3 wavelengths of photo energy or laser light, right into the localized area of inflammation in the tissue. Another modality we use is local cryotherapy which reduces inflammation of soft tissue using extreme cold to rapidly cool the skin surface which constricts blood vessels quickly forcing the fluid from inflammation away from the area. Click here to learn more about CryoTherapy.

Using both of these modalities in combination provides an extremely fast, safe and effective way to eliminate localized inflammation.

Biomechanical

The next phase of our treatment protocols involve the correction of mechanical stress that creates muscle spasm, myofascial adhesions or trigger points and joint dysfunction. To this end we utilize several therapies in concert to unwind the tension that builds in an individual, whether from a single trauma or simply the imbalance forces of daily wear and tear.

When there is considerable localized spasm or adhesions, we employ ShockWave therapy which is extremely effective in breaking up the spasm and adhesions. Shockwave therapy uses a pressure wave to break down the tissue tension, rapidly resolving the localized spasm and adhesions which helps to speed up return to the normal tissue tension. When muscle spasm and adhesions are less intense or inflamed, we incorporate the use of the Rapid Release Therapy Pro 3. This is a portable vibrational technology that quickly reduces muscle tension over the local areas where it’s applied. Unlike the percussive devices that have become popular in the last few years, the vibrational effect of the Pro 3 creates a much more comfortable and faster release of muscle tension. The challenge with percussive devices, and I used what I consider to be the top one on the market for 2 years, is that percussion, although effective at reducing tension, can be very uncomfortable when the tissue is inflamed and many patients can’t tolerate it so it becomes unuseful. However, with the Pro 3, since vibrational sensation travels to the brain faster than pain signals, I am able to deliver a much more comfortable treatment which helps to rapidly release the tension and speed up this phase of the protocol. The next phase of biomechanical correction involves a combination of both manual and technological applications geared to re-establishing proper muscular function of the muscles which were inflamed and/or in spasm, using a tool called a ProSoft. Through a process named Mechanical Motion Therapy, the ProSoft is used to reprogram a muscle to function at its normal contractile strength and resting length, helping to restore the proper range of motion of the joint(s) it supports and moves. This applies to not only spinal muscles but pelvic, hip, shoulder, and all other extremities that may be involved. For more info on the ProSoft, click here.

We also use a specialized Chiropractic table called the Lloyd Ultimate which allows us to mobilize spinal joints, particularly in the thoracic and lumbar spines using both passive and active motion to those areas to help restore normal spinal joint mechanics. The final component of biomechanical correction, which addressed joint dysfunction is spinal and extremity manipulation or “The Adjustment”. Through manipulative therapy, we can help restore the skeletal portion of the mechanics of weight-bearing and motion which is a vital piece of restoring neurological and musculoskeletal function.

All of these pieces used in combination is how we are able to very rapidly re-engineer an individual's dysfunction which is how the issue is resolved and the pain is removed. To reiterate the focus of the approach is not about treating pain. We are correcting the dysfunction that causes pain. When dysfunction is corrected and function is restored, the pain dissipates.

Now, this last piece is very important for us to understand as the practitioner but also, very important for the patient to understand for complete resolution to the issue. All of these processes have been geared toward restoring the tissues and mechanical system to normal. The body, which is always trying to stabilize itself moment to moment in weight-bearing and motion, has gotten to the point where that stability is dysfunctional, thus the pain syndrome. But there is stability nonetheless. When all of these corrections are made, there is a brief time of instability between the dysfunctional stability that existed prior to treatment and the functional stability that the treatment achieves. So now it's of utmost importance to reinforce the mechanical changes that we have provided so that the individual's neuromusculoskeletal system can now have control over that new functional stability. We utilize, as the final part of treatment, Whole Body Vibration in order to help the body more rapidly gain control over the newly restored functionality. A series of stretching and movements are done while on the vibration platform which rapidly causes muscle contraction and relaxation and while this is happening with range of motion exercises, the brain has gotten a fast-tracked control back of the motions and ranges previously not being utilized. Functional

The final piece to the anatomical puzzle involves correcting the weaknesses and imbalances of the muscles involved. The body has certain built-in mechanisms of stability and function that governs the way we bear weight and move. As stated earlier, sometimes either single incidences of trauma or uneven wear and tear that happens from daily tasks which, through the body’s normal compensation to the forces that act upon it, cause weaknesses and imbalances to occur. When these weaknesses and imbalances ultimately change the normal mechanism of weight-bearing and motion, it's just a matter of time before something stresses the system’s ability to withstand any additional stress. As an example, many people end up getting injured from some simple everyday task such as lifting a laundry basket or bag of groceries, bedding over slightly while brushing one’s teeth, or even bending over to put on a sock and other such mundanes tasks which result in that moment when everything locks up and the pain begins. It's not that these tasks are so difficult, it's just that the normal mechanics have been disrupted over time and that last task was enough to break the proverbial straw of the camel's back.

Now since the treatment has created change in the mechanical system of the individual, it must be reinforced so functional exercises are given as homework to the patient to reinforce on a more permanent basis, the change from dysfunctional stability to functional stability. By adopting these types of exercise on a regular basis, the patient now has a way to prevent the daily wear and tear to once again cause the dysfunction through the body’s compensation that led to pain in the first place and continue to remain pain-free and functional.

Maintenance

As with all machines, it is of great importance to maintain the system so that it continues to function optimally and not break down with time. All machines have a maintenance schedule, which if not adhered to, will nullify the warranty of that machine. When it comes to the body, I have seen very clearly for many years that those who come for the advanced Chiropractic treatment we deliver on a monthly basis, usually once every 3-4 weeks, are the people who never get into those pain syndromes again. So maintenance is key to prevent future occurrences of the existing or perhaps a new pain syndrome.
